1. The determination of the anesthesia fee is the sole right and responsibility of Precept Anesthesia Services, Inc.
2. Fee Schedules for anesthesia services will be provided to each dentist/physician contracting with Precept Anesthesia Services, Inc.
3. Payment for anesthesia services must be made at time of appointment/service rendered. A 50% down payment is required to hold your appointment.*
4. Precept Anesthesia Services, Inc. reserves the right to refuse anesthesia service in non-emergent situations if appropriate payment is not made.
5. Payment of the anesthesia fee may be made by cash or check. In some cases, visa or other credit plan is acceptable, as determined by Precept Anesthesia Services, Inc.
6. Payment receipts will be provided to the patient upon request
7. Precept Anesthesia Services, Inc. does not participate in any third party insurance programs. We will not be responsible for providing insurance claim forms to the patients. We will provide itemized receipts for services rendered and/or fill out any appropriate claim forms that we may be provided by the patient.

*A cancellation fee of $250.00 will be charged for failure to show or cancellations without thirty-six hours notice, or, as determined by Precept Anesthesia Services, Inc.
